Fill out Part I of form.  Enter your name as contact.  Include account number for payment and precise information on exact nature of work.
 
The Requestor's Signature line must be signed by individual with signature authority on account listed on form.  
 
Send form to Facilities Services (H7015) or fax to 471-8788
 
Facilities Services will work directly with person listed as contact.


Renovation Work Orders Estimated to Cost More Than $1,000

Facilities Services is not allowed to accept Work Orders with an estimated cost of more than $1,000 without prior approval from Dr. John Ekerdt, Associate Dean for Research.

Print form and fill out Part I.  List your name as contact.  It is imperative to include an account number to be used for payment of the estimate and renovation expenses.  Be sure to provide precise information on the exact nature of the work.
 

The Requestor's Signature line must be signed by Dr. John Ekerdt, Associate Dean for Research, if work is estimated to cost more than $1,000.
 

Forward the PP10 to Dr. John Ekerdt, Associate Dean for Research.  He will sign the form and send it to Facilities Services.  A work order number will be assigned and contact will be notified of that number.
 
A Project Manager will work with the contact listed on the PP10 and a Preliminary Scope Estimate will be produced. 

If you accept the estimate and want to proceed with the work, write a memo to Dr. John Ekerdt indicating acceptance of the Preliminary Scope Estimate.  It is critical that you respond within three months and identify the account to be used.  If Facilities Services does not receive a response within three months, the work order will be deleted.
 

Dr. Ekerdt will authorize funding for projects estimated up to $25,000.  He will work with the Provost's office to obtain funding authorization for projects estimated for more than $25,000.  Dr. Ekerdt's office will continue to work with Facilities Services and keep track of your work request.

he Cockrell School has established departmental representatives who coordinate contact with the Department Chairs, Dr. John Ekerdt, and Facilities Services for renovation projects. They have access to the WORQS Online Request forms and can submit your work order electronically. The representatives work with Facilities Services on a day-to-day basis and are familiar with renovation procedures. Please check with your department chairman's office for your representative's name.
