This form is to be used to request routine maintenance, custodial or non-emergency services ONLY.

Select New Request; select Service Request, fill out form and submit request to Physical Plant online

Fill out Part I of form. Enter your name as contact. Include account number for payment and precise information on exact nature of work.

The Requestor's Signature line must be signed by individual with signature authority on account listed on form.

Send form to Physical Plant, Planning & Scheduling (H7015) or fax to 471-8788

Physical Plant will work directly with contact.

Physical Plant is not allowed to accept Work Orders with an estimated cost of more than $1,000 without prior approval from your Department Chairman and Dr. Randall Charbeneau, Associate Dean for Research

Print form and fill out Part I. List your name as contact. It is imperative to include an account number to be used for payment of the estimate and renovation expenses. Be sure to provide precise information on the exact nature of the work.

The Requestor's Signature line must be signed by Dr. Randall Charbeneau, Associate Dean for Research, if work is estimated to cost more than $1,000.

Print out this form and attach it to PP10 form.

Send Work Order Tracking Form and PP10 form to your Department Chairman for his approval (signature) and priority ranking in Part I of Tracking Form.

Forward the two forms to Dr. Randall J. Charbeneau, Associate Dean for Research. He will provide a college-wide priority ranking in Part II of the Tracking Form and send the forms to Physical Plant�s Planning and Scheduling Office to have a work order number assigned. The contact will be notified of the work order number assigned by Physical Plant.

Physical Plant�s Architectural and Engineering department will produce a Preliminary Scope Estimate.

If you accept their bid and want to proceed with the work, contact your Department Chairman and request that he write a memo to Dr. Randall Charbeneau indicating his acceptance of the Preliminary Scope Estimate. It is critical that you respond within three months and identify the account to be used. If Physical Plant does not receive a response within three months, the work order will be deleted.

Dr. Charbeneau will forward the Chairman�s acceptance memo to Dr. Stephen Monti, Executive Vice Provost, for authorization of funding. Dr. Charbeneau�s office will continue to work with Physical Plant and keep track of your renovation project.

The College of Engineering has established departmental representatives who coordinate contact with the Chairmen, Randall Charbeneau, and Physical Plant for renovation projects. The representatives work with Physical Plant on a day-to-day basis and are familiar with renovation procedures. Please check with your department chairman�s office for your representative�s name. Contact your representative for assistance in preparing the PP10 form and attaching the Work Order Tracking Form.
